HOW TO REGISTER

Please use the ON-LINE REGISTRATION FORM to register for the 4th CECS.

After filling the personal details you will receive the password by the e-mail.

Upon receipt of the registration form and the payment, a Letter of Confirmation will be sent to the participants. Please present the Letter of Confirmation at the Registration desk at the Congress venue to pick up your materials.

Early registration fees will only be confirmed if the registration form and the payment are received by the Congress Secretariat on January 15, 2011at latest.

Registration forms received after April 20, 2011 will not be accepted as pre-registration. Please do not send any registration forms or payments after that date.

You will have the possibility to register in the Congress venue. The on-site registration will be available from April 27, 2011.

All participants will receive the Letter of Confirmation via e-mail or fax after the submission of their registration form.

 

CANCELLATIONS AND REFUNDS

The Congress Secretariat must be notified in writing about the cancellation of the registration. The appropriate refunds will be made after the Congress. The following cancellation conditions apply: 

Cancellations received by January 15, 2010

full refund of the registration fee less 30 EUR handling fee

Cancellations received on or after January 16, 2011: 

no refund

Name change

If you should be prevented from attending, you will be given the opportunity to send a colleague in your place. Name changes should be notified in writing to Congress Secretariat. A handling fee of 30 EUR will be charged for each name change.

 

METHODS OF PAYMENT

Registration fees must be paid in EUR by one of the following methods:

In case you use other cards (Diners Club or American Express) your payment will be charged in local currency – Czech crowns (CZK) at the exchange rate of the Czech National Bank on the date of payment. The approximate exchange rate is 1 EUR = 24, 637 CZK (November 2010).

Please use the On-line Payment System available after login or fill in Credit Card Authorization Form Fax it signed (Fax: +420-284 001 448) or send a scanned copy to the Congress Secretariat (cecs@guarant.cz).
